Licensing
To apply for a NSCMLT membership, please choose the appropriate application and forward the completed form to the NSCMLT office.
Initial Registration Application first time applicants (recent MLT graduates or to-be graduates please see the MLT Students page for additional required documents list)
for those previously licensed by the NSCMLT and wishing to reinstate after their license has lapsed
Internationally Trained If you are an Internationally Trained Professional, refer to the current NSCMLT Internationally Trained MLT Policy.
Return to Practice If you are planning to return to the work force after an absence of more than 5 years, refer to the current NSCMLT Return to Practice Policy.
